Unit owners may request maintenance to be completed by supplying American Properties Team, Inc. (APT) and the Board of Trustees' Chairperson the information listed on this form. Submit your request to APT via email, mail, fax or phone call. Using this standard form will assist APT and the Trustees to quickly understand your request and determine the appropriate action to be taken.
